We need an app orking in this way

1 – text field for sender's email 

2 – 2 check box (e.g. male/female) with the possibility to have both checked

3 – a text box to write a message (up to 1024 chars)

4 – a button to encapsulate text and checked box and send it to a server. (we will handle the server features)
