I need a referral form created in a SharePoint environment which can be accessed by all staff in the organisation. Details which are entered in the form are stored in a database which runs behind the form. The person who completes the form will be provided with on-going access to it. The Record of Work section will be completed by intervention workers who will resave the form after they have made their entries. The database will automatically update with each new entry made. Contents of the database can be dumped into an Access table or Excel spreadsheet.
